A few guidelines:

• Each Supplier and Input are aligned together
• Each Output and Customer are aligned together
• There is no need to align a Supplier-Input line with any specific Output-Customer entry
• This is best done by the following approach:
1. Identify process "boundaries"
2. Brainstorm Inputs
3. Identify Suppliers of Inputs
4. Brainstorm Outputs
5. Identify Customers for Outputs
6. Ask team if anything is missing
7. Complete the form
